Speech Language Pathologist

Conduct person-centered evaluations and treatment for residents with communication, cognitive, and swallowing disorders in a skilled nursing and rehab setting. Complete assessments, develop evidence-based care plans, and deliver individual and group therapy to improve functional outcomes and quality of life. Document thoroughly, participate in care conferences, educate families and staff, and contribute to continuous quality and clinical improvement initiatives within a collaborative, fast-paced environment.
