Job Summary
In-home Speech Language Pathologist (SLP) providing evaluations and treatments for adult and geriatric clients in Hudson County, NJ. Develop individualized treatment plans in collaboration with physicians and care teams, modify care plans as patient needs evolve, educate patients and caregivers to promote safety and independence, and complete timely documentation using a tablet-based EMR system. Collaboration with nurses, therapists, and interdisciplinary team members is essential in delivering compassionate, one-on-one home health care. Required Qualifications
- Current NJ Speech Language Pathologist license required
