Job Summary
Conduct home visits to clients in designated territories across Monmouth and Middlesex Counties, NJ, performing thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop treatment plans based on physician orders and goals, then continually revise care plans while participating in multidisciplinary care discussions. Educate patients and family members on rehabilitative activities to promote independent living, and accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
Required Qualifications
- A current license as a Speech Language Pathologist in STATE
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
- Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification
- Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification
Desired Qualifications
- Prior home care experience as a Speech Language Pathologist, SLP
- Solid computer skills
- Prior experience with electronic medical records (EMR)
