Job Summary
Provide one-on-one Speech Language Pathology to older adults wherever they call home (private residences, senior living communities, and related settings), including evaluations, individualized plans of care, and evidence-based treatment. Deliver therapy sessions to support wellness, mobility, and functional independence; document in FOX’s EMR per regulatory standards; collaborate with an interdisciplinary team; educate patients, caregivers, and staff to promote safety and aging in place; and provide telehealth/virtual care when appropriate to extend access and flexibility.
Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Current CPR certification
