Job Summary
Actify Home Care is seeking a Ge riatric Speech-Language Pathologist to provide one-on-one, field-based therapy in North Palm Beach County (Jupiter, Palm Beach Gardens, Riviera Beach, Juno Beach and surrounding areas). You will perform evaluations, re-assessments, and OASIS documentation, and develop and implement care plans addressing speech, language, and dysphagia (swallowing) disorders for geriatric patients, including stroke recovery and cognitive-linguistic care. Per-visit compensation is offered with electronic payments via Kinnser/WellSky, and clinical leadership supports documentation and caseload management.
Required Qualifications
- License: Active Florida Speech-Language Pathologist (SLP) License.
- Certification: Current CPR/BLS Certification.
- Transportation: Valid Driver’s License and reliable transportation.
- Software: Kinnser/WellSky experience is a major plus!
