Job Summary
Actify Home Care is seeking a licensed Florida SLP to provide one-on-one geriatric speech, cognitive, and swallowing therapy in patients' homes in Central Palm Beach County (West Palm Beach, Wellington, Royal Palm Beach, Riviera Beach, Lake Worth and surrounding areas). Responsibilities include completing evaluations, re-assessments, and OASIS documentation; developing and implementing plans of care addressing dysphagia, stroke recovery, and cognitive-linguistic needs; and delivering per-visit therapy with focus on geriatric populations. Must hold active Florida SLP license, CPR/BLS, valid driver's license and reliable transportation; experience with Kinnser/WellSky is a major plus.
Required Qualifications
- License: Active Florida Speech-Language Pathologist (SLP) License.
- Certification: Current CPR/BLS Certification.
- Transportation: Valid Driver’s License and reliable transportation.
- Software: Kinnser/WellSky experience is a major plus!
