Job Summary
Conduct home-based speech therapy for patients with communication, swallowing, or cognitive difficulties caused by illness or injury. Evaluate patient abilities in communication, swallowing, and information processing, then document progress and ensure established goals are met. Assist with mouth, breathing, and swallowing exercises, fluency, articulation, and reading while educating patients and families on care plans, therapy goals, and safety practices. Work as part of a multidisciplinary home health team including RNs, LPNs, CNAs, HHA, PT, and OT to support the patient's overall plan of care. Requires a Master's or Doctoral degree in Speech-Language Pathology, active Wisconsin SLP license, one year of speech therapy experience, and CPR certification.
Required Qualifications
- Master's or Doctoral degree in Speech-Language Pathology
- Active SLP license in Wisconsin
- Minimum of 1 year of speech therapy experience
- CPR certification
- Knowledge of state and federal home health regulations
