Job Summary
Provide home-based speech therapy to patients with communication, swallowing, or cognitive difficulties. Conduct patient assessments, document progress, and ensure goals are met while assisting with exercises for fluency, articulation, and reading. Observe and evaluate swallowing and processing abilities, then educate patients and families on safety practices and self-care. Work within a home health team including RNs, LPNs, and OTs to support the patient's plan of care. Maintain a 1:1 therapist-to-patient ratio and utilize a clinician-friendly EMR. Requires a Master's or Doctoral degree, active Wisconsin license, and one year of experience.
Required Qualifications
- Master's or Doctoral degree in Speech-Language Pathology
- Active SLP license in Wisconsin
- Minimum of 1 year of speech therapy experience
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgement
