Job Summary
Provide home-based speech therapy to patients with communication, swallowing, or cognitive difficulties due to illness or injury. Conduct patient assessments, document progress, and ensure established goals are met while assisting with exercises for mouth, breathing, swallowing, fluency, articulation, and reading. Work as part of a home health team including RNs, LPNs, CNAs, HHA, PT, and OT to support the patient's plan of care. Educate patients and families on therapy goals, safety practices, and self-care. This position is PRN with flexible assignments and 1:1 therapist-to-patient ratios.
Required Qualifications
- Master's or Doctoral degree in Speech-Language Pathology
- Active SLP license in Indiana
- Minimum of 1 year of speech therapy experience
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgement
