Develop and implement individualized treatment plans with functional, achievable goals, revising them based on patient performance and progress. Document evaluations, daily treatment, progress notes, and discharge summaries accurately and promptly while educating residents, families, and caregivers on therapeutic techniques. Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional abilities, participate in care conferences, and assist with discharge planning.
