Job Summary
Interpret medication orders to ensure safety, identify drug interactions, and coordinate solutions between pharmacy, nursing, and prescribers. Compounds and dispenses pharmaceuticals including sterile products, chemotherapy, and parenteral nutrition while verifying orders before delivery to patient care areas. Acts as a consultant to physicians and nurses on pharmacokinetics, adverse reactions, and therapeutic drug monitoring, adjusting therapy based on laboratory results. Manages pharmacy inventory, reviews restocking of automated dispensing cabinets, and completes prior authorizations and REMS fulfillment. Supervises ancillary personnel and technicians, trains staff, and performs final audits on prescriptions filled by technicians. Responds to emergency situations to assist with medication management and takes after-hours calls on a rotating basis to independently triage patient requirements.
Required Qualifications
- Active Clinical Pharmacist license in the state of Illinois (IDFPR)
- Bachelor's Degree in Pharmacy or a Doctor of Pharmacy Degree
- Must notify organization/department within one business day in the event of an adverse change of one's licensure or certification status
Desired Qualifications
- Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.)
- Completion of a pharmacy residency program or equivalent hospital pharmacy experience
