Specialty Pharmacist

Specialty Pharmacist to provide pharmaceutical care at a Community Health Center in Springfield, OH. Responsibilities include dispensing prescriptions, participating in pharmacy production workflow (insurance adjudication, filling, verification, packaging, dispensing), ensuring accuracy of prescription information and labels, balancing retail production with specialty patient needs, delivering clinical pharmacy services within a Patient Management Program, collaborating under protocols, providing clinical services and counseling for chronic conditions (e.g., asthma, diabetes, HIV/AIDS, IBD, autoimmune diseases, transplant), staying current with clinical guidelines, reviewing therapy for duplications and interactions, participating in Quality Management activities, and supporting medication access with Engagement Specialists. Requires a PharmD/BS in Pharmacy (or equivalent), Ohio licensure with ability to obtain multi-state licenses, 3+ years of direct patient care preferred, CPR/BLS required, immunization certification preferred, travel may be required.
