The Specialized Services Case Manager coordinates behavioral health and benefit services for residents of Nursing Facilities for Mental Health, including client intake, assessment, treatment planning, and Medicaid navigation.
Required Qualifications
- Associate’s degree plus two years’ experience in the health and human services field
- OR four years’ experience in the health and human services field
- Must complete Case Management Training and provide certificate within 6 months of hire
- Computer experience in Microsoft Office programs and basic computer fluency
