Specialized Case Manager - San Diego County

Job Summary

Manage a mixed caseload of Housing Access Services and other Catalyst program participants by processing referrals, expediting intake, and conducting assessments to develop individualized housing stability plans. Provide housing navigation, lease education, and eviction-prevention support while coordinating with landlords, housing authorities, and families to ensure tenancy stability. Supervise direct support staff through hiring, training, and performance evaluation, and prescribe individual training programs to meet client needs. Complete required logs, reports, and client evaluations while maintaining strict confidentiality and adhering to Title 17 standards. Perform direct service as needed and participate in the on-call rotation. Travel 75% of the time within San Diego County and state-wide quarterly for meetings and training.

Required Qualifications

  • 18 years of age or older
  • Bachelor's degree or three years equivalent experience and/or educational equivalent in working with individuals with disabilities
  • Prior successful experience as a Catalyst Case Manager or comparable case management experience supporting individuals with intellectual and developmental disabilities, housing stability needs, or community-based services
  • English and Spanish proficiency
  • Working knowledge of Title 17 Housing Access Services requirements, affordable housing resources, housing navigation practices, tenant education, and housing stability planning
  • Well-developed interpersonal skills
  • Demonstrated computer literacy (MS Office) and capability to maintain well-organized records and files
  • Ability to follow detailed, multi-step instructions
  • Ability to multi-task, work under pressure and manage time
  • Demonstrated ability to effectively navigate and maintain professional boundaries as custodian of confidential information
  • Certification in an approved First Aid and CPR class; or successfully complete an approved First Aid and CPR class within 45 days of the date of hire
  • Insured, registered vehicle in safe working conditions, and proof of current CA driver's license
  • Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time
  • Ability to travel in and throughout the state on a quarterly basis to attend meetings, training, etc.
  • Successful completion of pre-employment drug screen and background check
  • Acceptable DMV record
  • Mobile smart device (i.e. cell phone/tablet) with reliable internet connectivity and data
  • Ability to regularly stand, walk, sit, talk and hear
  • Capable of writing, using a computer keyboard, telephone and related office equipment
  • Ability to lift 25lbs
  • Ability to talk, hear, and see to assist the client
  • Must be able to react appropriately and quickly in the event of an emergency such as client elopement, a natural disaster, fire or other unforeseen circumstances
  • Appropriate and professional dress
  • Ability to work in the evenings or on weekends
  • Ability to perform direct service as needed
  • Ability to assist clients in and out of wheelchairs and other assistive devices
  • Ability to ascend or descend stairs
  • Ability to summon emergency responders

Desired Qualifications

  • Knowledge of American Sign Language (ASL) and/or Lengua de Señas Mexicana (LSM)
  • Prior successful experience as a Catalyst Case Manager or comparable case management experience supporting individuals with intellectual and developmental disabilities, housing stability needs, or community-based services preferred for promotional-level consideration
  • Working knowledge of Title 17 Housing Access Services requirements, affordable housing resources, housing navigation practices, tenant education, and housing stability planning preferred or required based on operational need
