Job Summary
Specialized Case Manager responsible for coordinating comprehensive behavioral health services for adults and children with mental illness and substance use disorders. Provides crisis counseling, short-term and center-based case management, case coordination, and medication refill assistance; assesses referrals based on clinician recommendations; travels locally to clients’ homes and training sites; ensures knowledge of available community resources and compliance with applicable laws; excels in delivering integrated mental health, substance use, and co-occurring services with strong communication and computer skills.
Required Qualifications
- Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field
- minimum of one year full-time or equivalent experience with individuals experiencing serious mental illness OR Bachelor's degree plus three years experience with such individuals
- travel locally to clients’ homes, training locations and other locations as necessary
- must complete approved mental health targeted case management training within three months
- knowledge of resources in the service area for adults with serious mental illness
- knowledge of and compliance with state and federal statutes, rules and policies affecting the target population
- ability to provide integrated mental health, substance abuse and co-occurring services
- ability to use a computer
- good oral and written communication skills
- ability to pass organization screening requirements including background screenings
