Specialized Case Manager - Imperial Valley

Job Summary
Manage a mixed caseload of Housing Access Services and other Catalyst program participants by processing referrals, expediting intake assessments, and developing individualized housing stability plans. Provide housing navigation services including lease education, tenant rights support, and eviction-prevention coordination while evaluating collateral data and assessing client needs. Effectively manage and develop direct support staff by overseeing hiring, training, and performance evaluations, and prescribe individual training programs. Collaborate with landlords, housing authorities, and community partners to coordinate participant-centered supports, ensuring compliance with Title 17 requirements. Complete all required logs, reports, and client evaluations while maintaining accurate documentation. Be part of the on-call rotation as needed and travel to job fairs, client meetings, and employer sites throughout the Imperial Valley region.
Required Qualifications
- 18 years of age or older
- Bachelor's degree or three years equivalent experience and/or educational equivalent in working with individuals with disabilities
- English and Spanish proficiency required whenever operationally feasible to support effective communication with the diverse communities served
- Certification in an approved First Aid and CPR class; or successfully complete an approved First Aid and CPR class within 45 days of the date of hire
- Insured, registered vehicle in safe working conditions, and proof of current CA driver's license
- Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time
- Ability to travel in and throughout the state on a quarterly basis to attend meetings, training, etc
- Successful completion of pre-employment drug screen and background check
- Acceptable DMV record
- Ability to regularly stand, walk, sit, talk and hear
- Capable of writing, using a computer keyboard, telephone and related office equipment
- Ability to lift 25lbs required
- The Specialized Case Manager must be able to perform direct service as needed
- Must be able to react appropriately and quickly in the event of an emergency such as client elopement, a natural disaster, fire or other unforeseen circumstances
- The Case Manager is expected to perform duties assigned during regular working hours
- As necessary, the Case Manager may be called upon to work in the evenings or on weekends
- Overnight travel may be required in the performance of the Case Manager's job duties
Desired Qualifications
- Knowledge of American Sign Language (ASL) and/or Lengua de Señas Mexicana (LSM) highly preferred to strengthen accessibility and inclusive service delivery
- Prior successful experience as a Catalyst Case Manager or comparable case management experience supporting individuals with intellectual and developmental disabilities, housing stability needs, or community-based services preferred for promotional-level consideration
- Working knowledge of Title 17 Housing Access Services requirements, affordable housing resources, housing navigation practices, tenant education, and housing stability planning preferred or required based on operational need
