Conduct day-to-day support for self-located individuals with mild intellectual disabilities who have come into contact with the justice system. Develop, execute, and evaluate support plans while guiding clients on housing, finances, administration, employment, and daily structure. Stimulate self-reliance and autonomy to facilitate reintegration into society by coordinating with family, reclassification services, and other stakeholders. Participate in multidisciplinary meetings to discuss complex cases and contribute to quality improvement. Maintain contact with clients in offices, at home, or within penitentiary institutions, working independently with flexibility in scheduling.
