Provide end-responsible specialist medical care for older residents with and without intellectual disabilities across multiple Cordaan locations. Lead intensive intramural multidisciplinary collaboration and establish external connections with hospital partners and other stakeholders. Contribute continuously to evidence-based practice improvements and participate in Cordaan's policy development initiatives. You will work within the specialist geriatric medicine group, engaging in geriatric rehabilitation, long-term care, and extramural services. Join annual group meetings including sessions with the Board of Directors and support the annual training of three to four AIOS.
