Job Summary
Specialist, IRIS Consulting (Milwaukee County, WI) is a remote/home-based role supporting Wisconsin IRIS Program participants. You will build relationships with participants to help them navigate the IRIS program, identify personal outcomes, and develop customized IRIS plans covering goals related to employment, housing, health, safety, community membership, transportation, and lasting relationships. The role requires meeting in person with IRIS participants a minimum of four times per year (one annual home visit), documenting activities in the IRIS data system (WISITs) within 48 business hours, educating participants on budget reports, and coordinating with the Fiscal Employer Agency and IRIS Consultant Agency. You will work with participants to ensure they operate within their IRIS budgets, explore community resources, maintain confidentiality and HIPAA compliance, and collaborate with staff to implement participants' plans. Regular in-person monthly team meetings and weekly check-ins are part of the duties. The position emphasizes building positive relationships, self-directed program navigation, and a strong focus on high-quality work across diverse settings.
Required Qualifications
- At least 2 years experience in health care, preferably in care coordination
- At least 1 year of experience serving target groups of the IRIS program (adults with intellectual/physical disabilities or older adults)
- Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or closely related field (or four years of commensurate experience if no degree)
- Valid and unrestricted driver’s license, reliable transportation, and adequate auto insurance for job related travel requirements unless otherwise required by law
