Job Summary
Assess referred clients' medical and psychosocial histories to identify service needs, then provide interventions and referrals for medical, psychosocial, and basic needs resources. Create patient-centered care plans, conduct home or site visits to identify needs, and educate patients on available healthcare services. Document ongoing follow-ups and services in EHRS records within 72 hours, complete monthly reports, and monitor service efficacy through periodic re-evaluations. Collaborate with Legacy staff, managed care organizations, and community agencies to facilitate access to social supports while advocating for clients to reduce service gaps. Work within a primary care medical home framework, maintaining a clean workspace and attending required training. Monday-Friday (8AM-5PM) at Legacy Montrose Allen Parkway in Houston, TX. Community Health Worker certification required; Bachelor's degree preferred.
Required Qualifications
- High School Diploma or equivalent
- Community Health Worker (CHW) Certification
- Minimum of 5 years of relevant experience
- 1 year of experience working with families, assessing psychosocial and health needs, and making community referrals
