Specialist Case Management HIV

Conduct case management and coordination for patients and families requiring educational, social, and advocacy services. Research and deliver educational materials tailored to specific population needs. Assist individuals in developing support systems and link them to community resources. Collaborate with staff, payers, and external partners to identify needs, cultivate relationships, and enhance clinical outcomes. Some employees may operate a motor vehicle to conduct business. This role supports the broader mission of MaineGeneral Health by providing education, advocacy, and support to patients and families.
