Job Summary
Social Worker in Agile Psychological Medicine (aPM Clinics) at Monash Health delivering timely, evidence-based mental health care within a multidisciplinary team of Psychologists, Social Workers, and Psychiatrists. Conduct comprehensive psychosocial assessments for consumers referred through psychiatric triage, emergency departments and inpatient services; develop recovery-focused treatment plans with consumers; provide therapeutic social work interventions and system-based support to improve outcomes; utilise routine outcome measures to inform clinical decision-making; contribute to a supportive team and continuous improvement in care.
Required Qualifications
- Experience working within mental health settings
- Strong assessment, engagement, and therapeutic intervention skills
- An interest in systems-based and/or family-inclusive practice
- The ability to work independently while contributing effectively within a multidisciplinary team
- A flexible and adaptable approach to managing a dynamic clinical workload
- A commitment to evidence-based practice and using outcomes to inform care
