Special Projects Lead, Strategic Clinical Programs

Job Summary
Own the clinical program menu end to end for orthopedics, cardiovascular, and oncology, defining target patients, protocols, and staffing models. Execute delivery for health system partnerships, managing bespoke programs and live reporting for at-risk and value-based arrangements. Build outcome evidence to scale pilots into referral pipelines, establishing KPIs for re-hospitalization, margins, and patient outcomes. Work directly with Partnerships, Clinical, and hospital leadership to design, launch, and optimize programs across regions. This role blends clinical design with commercial rigor to drive market share in the home health sector.
Required Qualifications
- A clinical background. RN, therapist (PT/OT/SLP), NP, PA, MD, or a clinical operations leader who has run clinical delivery
- 3-5 years owning complex, ambiguous projects end to end, where you had many stakeholders and no playbook, and you delivered
- Analytical rigor. Comfort with data analysis (SQL, Excel modeling, Claude Code) to build a business case, set a baseline, and defend a number to a partner who is motivated to argue with it
- Exceptional written and verbal communication
- Track record of owning outcomes, not just output. Work that demonstrably moved patient outcomes, volume, cost, or revenue
- Operator mindset: you take a vague ask and return something excellent without needing it fully specified
- Accountable end to end: you unblock yourself, escalate when necessary, and iterate quickly
- Clinically grounded, commercially sharp: you can hold a real conversation about heart failure readmissions and about the margin on the contract that pays for it
- Comfort with ambiguity: you can operate zero to one
- Low ego, high standards: this is a startup. You get scrappy, you sweat the details, and you hold the line on quality and on what we are willing to promise
Desired Qualifications
- Experience building or running a service line (ortho, cardiac, oncology) at a health system, or standing up a clinical program at a provider organization
- Home health, post-acute, or hospital-at-home experience
- Exposure to value-based care, at-risk contracting, bundled payments, or CMS models
- Experience with health system partnerships or JVs
- Background in management consultant
- Comfort with telehealth-enabled care models
