Spanish Speaking Family Medicine/Internal Medicine MD/DO

Provide in-patient and out-patient comprehensive primary care services, including preventive, acute, and chronic care, alongside clinical evaluation, diagnosis, and treatment. Adhere to disease management protocols and key clinical pathways to improve performance, while facilitating referrals for social services and participating in staff outreach and off-site health care programs. Rotate to other CFHC centers as needed for coverage and provide off-hours and Saturday clinic coverage as assigned. Work cooperatively with the clinical team and outside specialists, ensuring compliance with federal and state government-approved programs.
