Job Summary
Support clients with cognitive, physical, and psychological impairments in their daily lives and leisure activities within their homes and families. Create individualized integrated participation plans (PiT), digital assistance plans, and manage documentation while coordinating with legal guardians, relatives, institutions, therapists, and doctors. Independently schedule appointments in consultation with the team leadership and colleagues. The role is full-time or part-time (60-80%) with a starting salary of €4,417 or €4,770 inclusive of the SuE allowance, rising to €5,634 with extensive experience. Candidates require a degree in Social Pedagogy or Social Work, relevant experience, and a driver's license. The position offers a hybrid work option, internal training programs, and a monthly shopping card worth up to €50.
Required Qualifications
- Abgeschlossenes Studium der Sozialpädagogik, der Sozialen Arbeit, der Heilpädagogik oder eine vergleichbare Qualifikation
- Berufserfahrung in der Arbeit mit Menschen mit kognitiver Beeinträchtigung
- Vertrautheit mit dem BTHG
- Vertrautheit mit dem Rahmenvertrag 3
- Vertrautheit mit dem PIT
- Besitz eines PKW-Führerscheins
- Beherrschung gängiger PC-Anwendungen
