Job Summary
Conduct home-based outreach to support individuals with disabilities, mental health conditions, and substance use disorders in the Gütersloh area. Lead relationship-building through conversations with clients, therapists, and family members to identify personal resources and strengthen daily functioning. Coordinate care by organizing necessary assistance, facilitating access to specialists, and managing hospital stays to promote social participation and independent living. Assist clients in establishing stable life foundations and daily routines while maintaining empathy and flexibility for diverse needs. Requires a degree in social work, psychology, or related fields, knowledge of social security laws, and a Class B driver's license.
Required Qualifications
- Abgeschlossenes Studium: Sozialarbeiter / Sozialpädagoge / Psychologe / Erziehungswissenschaftler / Heilpädagoge
- Fachliches Engagement: Bereit, Dich auf schwer erkrankte Menschen im häuslichen Umfeld einzulassen
- Flexibilität & Empathie: Kannst Dich auf wechselnde Bedarfe einstellen und begegnest Menschen mit Mitgefühl und Offenheit
- Rechtliches Know-how: Kenntnisse in den Sozialgesetzbüchern V, VIII, IX und XI
- Führerschein Klasse B
- Bereitschaft, die diakonische Ausrichtung unserer Arbeit aktiv zu unterstützen
