Conduct soziotherapeutic accompaniment and support for individuals with multiple diagnoses, guiding them in daily life management and maintaining daily structure. Develop, plan, and execute group and individual activities while stabilizing abstinence and psychological well-being. Create reports including the person-integrated participation plan (PIT) and document progress. Collaborate with statutory guardians and payers, participate in team meetings, and engage in supervision. This role supports the mission of the "Soziotherapeutischen Zentren Heide" to improve care for dependent individuals across open, closed, and residential facilities in Darmstadt. Full-time or part-time positions are available immediately.
