Conduct patient and family counseling regarding social, economic, and personal matters while coordinating with municipal offices and social service agencies. Manage discharge planning by assessing and monitoring post-discharge needs, and participate in interdisciplinary rounds and medical meetings to foster cross-functional collaboration. Require a degree in Social Pedagogy or Social Work, preferably with clinical social work experience, alongside strong communication skills and organizational ability.
