Job Summary
Assess psychosocial needs, strengths, and limitations of current, new, and prospective residents while addressing their emotional concerns and goals. Develop, implement, and update individualized psychosocial care plans, then coordinate services with interdisciplinary teams including physical therapy to ensure continuity of care. Provide counseling, emotional support, and resource referrals as needed, maintaining ongoing communication with residents, families, and case managers. Ensure compliance with all facility policies, regulations, and documentation standards. Full-time and part-time roles available; Bachelor's degree required with Master's preferred and minimum one year of healthcare social work experience.
Required Qualifications
- Must be able to work full-time hours and maintain a flexible schedule
- Bachelor's degree required
- Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required
- Strong knowledge of regulations, policies, and guidelines governing nursing care facilities
- Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies
- Ability to work effectively in a fast-paced, team-oriented healthcare environment
Desired Qualifications
- Master's degree strongly preferred
- experience in a nursing home or rehabilitation center strongly preferred
