Job Summary
Evaluate psychosocial needs of current, new, and prospective residents while developing individualized care plans and coordinating services with interdisciplinary teams. Address resident concerns and emotional needs through direct counseling, resource referrals, and ongoing communication with families and case managers. Build supportive relationships with residents and ensure compliance with facility policies and documentation standards. Participate in care planning meetings to support continuity throughout the resident's stay. Requires Bachelor's degree with 1+ years of healthcare social work experience and full-time flexible scheduling.
Required Qualifications
- Must be able to work full-time hours and maintain a flexible schedule
- Bachelor's degree required
- Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required
- Strong knowledge of regulations, policies, and guidelines governing nursing care facilities
- Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies
- Ability to work effectively in a fast-paced, team-oriented healthcare environment
Desired Qualifications
- Master's degree strongly preferred
- experience in a nursing home or rehabilitation center strongly preferred
