Job Summary
Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families. Deliver counseling, crisis intervention, and bereavement support, including follow-up for at-risk individuals and coordination of bereavement activities. Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end-of-life needs, and liaise with community agencies while ensuring HIPAA compliance. Requires a Bachelor's degree in social work, current five-level licensure (CSW, CAPSW, CISW, LCSW, SWTC), and ability to pass background and drug tests. Schedule is M-F 8:00am to 5:00pm, full-time.
Required Qualifications
- Bachelor's degree in social work from an institution accredited by the Council on Social Work Education
- Current Social Work licensed in the state(s) of practice
- Certified Social Worker (CSW)
- Certified Advanced Practice Social Worker (CAPSW)
- Certified Independent Social Worker (CISW)
- Licensed Clinical Social Worker (LCSW)
- Social Work Training Certificate (SWTC)
- Ability to pass background, consumer report, and drug test
- Reliable transportation
- Valid driver's license
- Proof of auto insurance
