Job Summary
Conduct social work assessments to determine psychosocial needs and actively participate in interdisciplinary team meetings to develop participant care plans. Deliver and document interventions including arranging resources, assisting with care transitions, providing counseling, and managing cases. Partner with primary care physicians to guide smooth transitions between settings and initiate care conference meetings to ensure coordination among team members and support networks. Provide discharge planning when participants disenroll from the program. Maintain a caseload while collaborating closely with clinical and non-clinical members to keep frail seniors living safely in the community.
Required Qualifications
- Master's degree in social work (MSW)
- CPR and First Aid certification
- Proof of vaccination or a vaccine declination form
Desired Qualifications
- Minimum of one (1) year of experience working with a frail or elderly population
- Experience coordinating and facilitating care conference meetings
- Experience assisting people with behavioral health or substance abuse issues
- CPR and First Aid certification upon hire
