Job Summary
Social Worker responsible for conducting assessments, preparing and reviewing care plans, and facilitating team discussions to develop Plan of Care. Provides ongoing support and education to participants and families on aging, dementia, grief and loss, end-of-life, and related disease processes; makes referrals to contracted facilities, behavioral health, and community agencies; initiates external referrals (e.g., Adult Protective Services, Housing Authority, Colorado Springs Police Department). Requires a Master's in Social Work and at least two years of experience in skilled rehabilitation, with a preference for multidisciplinary-team collaboration. Proficiency in Microsoft Word, Excel, and Outlook. Location: Colorado Springs, CO with on-site responsibilities.
Required Qualifications
- Masters Degree in Social Work
- A minimum of two year of experience working with a skilled rehab.
- Experience working on a multidisciplinary team preferable
- Familiarity with Microsoft applications such as Word, Excel and Outlook
