Job Summary
Provides social work services to all age groups of patients and significant others to facilitate adjustment to illness and maximize benefit from healthcare services; coordinates with community resources (long-term care, home health) and collaborates with medical staff; maintains documentation of assessments and interventions; ensures HIPAA and regulatory compliance; supports discharge planning and connects patients/families to appropriate services. Requires knowledge of tri-state community resources and ability to communicate effectively in a hospital setting; position is part-time with 24 hours weekly.
Required Qualifications
- BSW degree or equivalent in Social Work
- Part-Time schedule (24 hours weekly)
- Ability to communicate effectively with patients, families, and hospital staff
- Knowledge of HIPAA and relevant laws/regulations
- Experience or training in medical social service practice and discharge planning
