Job Summary
Adult Social Worker for the Young People Transitions to Adulthood Team based in Manchester. Responsibilities include supporting young people preparing for adulthood, conducting holistic assessments of aspirations, needs, risks, and required support as they transition from children’s to adult services; coordinating multi-agency transition plans with education, health, and social care to ensure continuity of care and smooth handover; promoting independence and person-centred outcomes by developing life skills and wellbeing plans to empower decision-making. Required Qualifications
