Job Summary

Assess hospice patients to identify psychosocial, financial, environmental, and community needs through documentation, clinical records, and team meetings. Facilitate communication among team members to achieve desirable outcomes while promoting agency philosophy and administrative policies. Participate in interdisciplinary team meetings and collaborate on patient care planning. Accept clinical assignments consistent with education and competence to meet patient needs. Maintain mandatory continuing education and public/private eligibility standards. Requires a Bachelors or Master's in Social Work, Texas Certification, and one year of healthcare agency experience. Works in patient homes with occasional exposure to infectious diseases and unpleasant weather.

Required Qualifications

  • Bachelors or Master's Degree in Social Work from a state accredited program
  • Texas Certification
  • Texas Driver's License
  • One (1) year social worker experience in a healthcare agency
  • Working knowledge of community resources
  • Good interpersonal skills
  • Hepatitis profile
  • Reliable transportation
  • Valid auto liability insurance
  • Ability to travel locally
  • Ability to work under some stressful conditions
  • Ability to make quick decisions
  • Ability to acquire resources
  • Ability to meet family/patient psychosocial needs
  • Ability to assess hospice patient to identify psychosocial, financial, environmental, and community needs
  • Ability to use effective interpersonal relations and communication skills
  • Ability to facilitate the use of these skills by other team members
  • Ability to maintain a working knowledge of public and private eligibility standards and requirements
  • Ability to promote Agency philosophy and administrative policies
  • Ability to attend interdisciplinary team meeting
  • Ability to participate in the patient care planning process
  • Ability to collaborate with Interdisciplinary Team
  • Ability to accept clinical assignments consistent with education and competence
