Job Summary
Coordinate admission and discharge paperwork, create and amend care plans, and monitor the psycho-social needs of facility residents. Identify required social services, manage Medicaid and Medicare documentation, and lead discharge planning for each resident. Conduct MDS interviews and social services assessments while balancing caseloads and charting. Coordinate multi-disciplinary care conferences, assist with room changes, and educate clients on end-of-life options. Maintain infection control standards, respond to resident concerns with confidentiality, and advocate for resident rights. Lead the TCU quality committee, complete guest satisfaction surveys, and participate in training to stay current on regulations.
Required Qualifications
- Degree in Social Work
- Current MN Licensure in Social Work
- ability to successfully obtain license within 60 days
- Proficiency in Microsoft Office
- digital literacy for charting and interfaces with outside services
- Ability to pass NetStudy 2.0 background screening
