Job Summary
Conduct psychosocial assessments and develop individualized intervention plans for residents, families, and caregivers to enhance coping with illness, loss, and placement. Facilitate therapeutic groups, resource navigation, and peer support while coordinating referrals to government agencies and community services. Provide discharge planning, follow-up services, and legal documentation assistance, including applications for financial benefits and guardianship. Maintain accurate records, complete monthly statistical reports, and participate in interdisciplinary care conferences and team rounds. Collaborate with the multidisciplinary team to implement and evaluate programs, ensuring alignment with ACSW Standards of Practice.
Required Qualifications
- Bachelor's Degree in Social Work
- At least two years of recent experience in related Social Work
- Registered and in good standing with the Alberta College of Social Workers (ACSW)
- Able to work effectively in a physically and mentally demanding environment
- Demonstrates an acceptable level of reading, writing, and comprehension of English language skills
- Excellent interpersonal skills
- Strong verbal and written communication skills
- Must possess initiative
- Good judgement
- Demonstrate the ability to work independently with minimal supervision
- Successful candidates will be required to obtain a Criminal Background Check
- Successful candidates will be required to provide proof of all mandatory immunizations
