Job Summary
Social Worker (LMSW) at DaVita in an outpatient dialysis setting. Build meaningful relationships with patients and families, advocate for patients, and guide them in managing the physical, mental, emotional, and financial aspects of End Stage Renal Disease. Responsibilities include helping patients understand their rights and responsibilities, coordinating care within the patient’s core care team, and supporting patient-centered care. Requires LMSW in TX and MSW with clinical specialization; comfortable with insurance programs and government/private resources; proficient with MS Word/Excel/PowerPoint/Outlook and DaVita applications. Per Diem with potential for full-time, schedule starts at 2 days per week, no weekends, daylight hours; emphasis on belonging, teamwork, and comprehensive benefits.
Required Qualifications
- LMSW required
- Master's degree in Social Work (MSW) required with a specialization in clinical practice
- Meets all state required regulations to practice in the dialysis setting
- Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
