Job Summary
Provide counseling, information on community support access, assistance with financial needs, and education on cancer and chronic disease treatment to patients and family members. Serve as the liaison between patient/family and medical/clinical staff, community agencies, and internal departments. Conduct assessments, diagnosis, treatment planning, and case management while maintaining a community resource database through effective liaisons with key assistance sources. Possess skills in building and maintaining these relationships to support the hospital's personalized care model for the 250,000 residents in 17 local counties.
Required Qualifications
- Bachelor's degree in Social Work
- Current Montana License, eligibility for licensure, or working on hours for licensure
- Knowledge of social work principles and practices, including current standards within the field
- Ability to use a variety of diagnostic tools and techniques including DSM IV-R
- Knowledge of the development and maintenance of information/referral systems
- Skills in assessment, diagnosis, treatment, counseling, and case management
- Oral and written communication skills
- Time management skills
Desired Qualifications
- Master's degree in Social Work
- Background in working with cancer patients, or other type of critical care area
