Job Summary

Conduct psychosocial assessments, counseling, and discharge planning for patients in surgery, medicine, and pediatrics under supervision. Participate in interdisciplinary planning, unit meetings, and Rusk evaluation conferences while coordinating with care managers to ensure effective throughput. Mobilize community agencies and support networks, document activities per protocols, and supervise interns when eligible. Prepare statistical documentation and present psychosocial information during rounds. Assist in orientation for new staff and conduct patient/family education groups regarding diagnosis, treatment, and post-hospital care options.

Required Qualifications

  • Masters Degree in social work, or its equivalent (e.g. a higher degree from a social work program that is registered by the New York State Education Department as qualifying for licensure), from a recognized college or university program accredited by the Council on Social Work Education (CSWE)
  • New York State licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) or a New York Education Department-issued limited permit
  • Lic Clinical Social Worker
  • Lic Social Worker - NYS-Permit
  • Licensed Social Worker - NYS
  • three years post-graduate, SIFI eligible

  • Previous experience in a hospital or health-related organization
