Job Summary
Social Worker to assist patients in the Grandstrand area with home and facility visits (ALF/ILF) and coordination of advance care planning, assessments, and placement. Responsibilities include initiating advance care planning for patients, completing daily comprehensive assessments and care plans, documenting chronic care management time, meeting daily productivity goals, coordinating discharges and placement with contracted facilities, maintaining relationships with referral sources, and ensuring clear communication among patients, families, and care teams. Travel is required daily; role operates during full-time, 8am–5pm, Monday–Friday in Myrtle Beach and surrounding areas.
