Job Summary
Provide Selfhelp Active Services for Aging Model (SHASAM) services to residents, focusing on housing stability and reducing social isolation through case management. Responsibilities include conducting intakes, providing referrals, assisting clients with applications, advocating for clients, and maintaining communication with clients and families. Must prepare case notes, participate in meetings, and facilitate group activities. Requires a Bachelor's Degree and strong communication and customer service skills.
