Job Summary
Social Worker role in South Bristol Community Links Team. Manage a varied caseload across community-based adult social care services, undertaking Care Act assessments and reviews, safeguarding enquiries (Section 42), Mental Capacity Assessments and Best Interest decisions, and duty rota responses. Involves Court of Protection work where required, facilitating hospital discharges (including forensic and community mental health cases), and completing assessments for out-of-area placements. Collaborative work with partner agencies, maintaining accurate case records and care plans. Requires significant experience in Adult Social Care, strong knowledge of the Care Act and Mental Capacity Act, and excellent communication/organizational skills. Hybrid working model with initial higher office presence before transitioning to hybrid arrangement; minimum 2 days per week in the office.
Required Qualifications
- Significant experience within Adult Social Care
- Ability to manage a varied caseload independently
- Strong working knowledge of the Care Act and Mental Capacity Act
- Experience in safeguarding and statutory responsibilities
- Experience using LAS is desirable
- Excellent communication and organisational skills
