Social Worker / Social Work Executive (FSCPG)

Manage 25 case and counselling active cases per quarter, conducting assessments, planning, and interventions while ensuring proper documentation and timely report submissions. Conduct case conferences with partners and participate in networking and case management meetings. Manage case referrals as part of the Intake & Triage Team, performing intake assessments based on Code of Social Work Practice standards and referring clients to appropriate agencies. Network with other organizations, implement community outreach initiatives, and contribute to the programme team for Group Work at the FSC. Requires a Degree or Graduate Diploma in Social Work with understanding of social work values and ethics.
