Social Worker – Short Breaks Service with Children with Disabilities

Manage a complex and varied caseload by completing Short Break Assessments and Carer Assessments while ensuring arrangements are regularly reviewed with robust risk analysis. Maintain accurate Care Plans and Reviews, undertake direct work with children and families, and provide effective support within relevant statutory frameworks. Use professional judgement and evidence-based practice to inform complex decision-making, work closely with multi-agency partners, and contribute to safeguarding children through effective assessment, planning, and intervention. Prepare high-quality reports, maintain accurate case records, and support the continuous development of practice within the team.
