Job Summary
Engage referred Veterans in services, perform eligibility assessments for VA health care and VASH vouchers, and conduct crisis interventions for immediate needs. Complete assessment and treatment plans, assist with housing Authority applications and voucher issuance, and utilize a Housing First approach to ensure rapid placement. Provide face-to-face mental health interventions and therapy consistent with designated Levels of Care, establish intensive therapeutic relationships, and formulate case-management goals. Collect outcomes data, document clinical services in the VA Electronic Health Record, and coordinate interventions with the Veterans Administration and local community resources.
Required Qualifications
- BBS registered (or registration-eligible) ACSW/AMFT
- one years' experience in case management within a social service position
- clean DMV record
- reliable transportation
- ability to drive and transport
- complete government background check
- finger print and clearance
- Knowledge of or experience with DSM diagnoses
- assessment of level of functioning
- EBP's such as Motivational interviewing
- Critical Time Intervention
- DBT
- Harm Reduction
- Cognitive Behavioral Therapies
- Trauma Informed Care
- Housing First
- PTSD
- Ability to work collaboratively with others and on a team
- Ability to work independently

Desired Qualifications
