Job Summary
Provide emotional support and counseling to patients and families navigating end-of-life care. Complete psychosocial assessments and develop individualized plans to meet each patient's unique needs. Connect patients and families with community resources, advance care planning, and supportive services. Serve as an advocate, educator, and trusted guide throughout the hospice journey. Collaborate with nurses, physicians, chaplains, and hospice aides to provide exceptional, patient-centered care. This role requires a BSW or MSW, current Missouri licensure, and one year of healthcare experience. Flexible 20-hour weekly schedule with competitive hourly pay.
Required Qualifications
- Bachelor's or Master's degree in Social Work (BSW or MSW)
- Current Missouri Social Work license (or eligibility for licensure, as applicable)
- Minimum one (1) year of healthcare experience (hospice or home health preferred)
- Excellent communication, organization, and relationship-building skills
- Current CPR certification (or willingness to obtain)
- Valid driver's license, reliable transportation, and automobile insurance
