Job Summary
Evaluate residents to assess psychosocial needs, strengths, and limitations, then develop and update individualized care plans. Address resident concerns and emotional needs while maintaining communication with families and case managers. Build supportive relationships with residents and collaborate with interdisciplinary teams including physical therapy. Coordinate care and participate in care planning meetings to ensure continuity throughout the stay. Provide counseling, emotional support, and resource referrals as needed. Ensure compliance with all facility policies, regulations, and documentation standards. Work full-time hours with a flexible schedule.
Required Qualifications
- Must be able to work full-time hours and maintain a flexible schedule
- Bachelor's degree
- Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting
- Strong knowledge of regulations, policies, and guidelines governing nursing care facilities
- Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies
- Ability to work effectively in a fast-paced, team-oriented healthcare environment
