Job Summary
Conduct social assessments and develop individualized care plans to address the social, emotional, and spiritual needs of residents. Provide counseling, guidance, and support to residents and their families while collaborating with healthcare professionals to implement comprehensive care strategies. Advocate for residents' rights and maintain accurate records of interactions and care plans. Participate in facility-wide committees to improve resident care services.
Required Qualifications
- Bachelor's degree in Social Work or related field
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
- Excellent communication, interpersonal, and problem-solving skills
